My son wants to have sex with his mother, should I say something?

I found my son's journal and I read it. He's 14. He described what he'd like to do with my wife, his biological mother, in graphic detail. I'm shocked and disgusted. Should I confront him? Should I say anything to my wife? I can't get this out of my head. He's seriously disturbed.
